The process of lap band surgery in Chicago begins with registering for a free informational seminar at Westlake Hospital in Melrose Park, Illinois. Our free seminar will give you a chance to meet our Team and ask questions about our Chicago lap band surgery program. - Health History Information Sheet
- Information Sheet (if not handed in at the information session).

- Medical record documentation of past medically supervised weight loss attempts and co-morbid conditions from other physicians that you have seen the past five (5) years.
After meeting with Chicago lap band surgeon Dr. Frederick Tiesenga and our Bariatric Nurse Program Coordinator, our staff will assist you in navigating the following steps and insurance requirements within our Chicago lap band surgery program. These steps may include the following but are not limited to:
- Submission of insurance requirements and documentation.
